Farm Service Agency (FSA) Forms
If you are a new and beginning farmer and want to take advantage of USDA programs like low-interest operating loans, cost-share for conservation practices, and crop insurance, the place to start is at your county Farmer Service Agency (Georgia FSA map). This is where you’ll set up a profile as a farmer and receive a farm number for your operation, which will be used across many USDA agencies and programs.
Here are a few important forms that FSA requires when you get started:
AD-2047 — Customer Data Worksheet
If you are a socially disadvantaged producer and believe you are eligible for potential debt forgiveness through the American Rescue Plan, you must have a correct AD-2047 on file with your local USDA Farm Service Agency office.
AD-1026 — Highly Erodible Land Conservation (HELC) and Wetland Conservation (WC) Certification
